ALL Things Through Health offers patient advocacy services through the MyHealth Advocate program for those who may need additional help with medication management, healthy food choices, appointment coordination, along with continuity and transition of care services.
The MyHealth Advocate program is offered to people who have the following conditions: Pre-diabetes/diabetes, high blood pressure, asthma, and congestive heart failure (CHF).

How Does the Program Work?

Our advocate managers are registered nurses (RNs) or licensed practical nurses (LPNs). Once a free consultation is requested, a team member will contact you within 24-48 hours to set up the assessment by phone, in-person, or virtually.

Medication Review and Coordination
- Review of medications (in-depth)
- Provide ongoing monitoring
- Discuss new medications added after initial assessment
- Help with medication adherence – (staying on track with taking medications)
- Consult with clinicians and pharmacists on patient behalf
Healthy Food Choices/Management
Healthy Food Choices/Management
- Discuss best food choices to manage chronic disease(s)
- Coordinate referral to nutritionist
Appointment Coordination and Accompaniment Upon Request
Appointment Coordination and Accompaniment Upon Request
- Arrange transportation to medical visits
- Coordinate specialist referrals
- Provide in-patient beside monitoring
- Discuss and clarify clinician notes and plan of care with patient
- Review discharge plan with patient prior to release
- Provide companion to follow-up visits
Continuity of Care

- Advocate for patient with providers (e.g., clinicians, pharmacists, DME providers) as needed
Transition of Care
Transition of Care
- Facilitate safe discharge to home, skilled facility, rehab, or hospice
- Assist patient with coordination of services
- Advocate for needed services (e.g., supplies, equipment, etc.)
- Educate and inform about community resources available

There are out-of-pocket fees for the MyHealth Advocate program. However, the consultation is free of charge.
